What is Transitional Care?


Transitional Care—formerly known as Swing Bed—is a patient-centered program designed for individuals who no longer need acute hospital care, but still require skilled nursing or rehabilitative therapy before safely returning home.

This service provides the bridge between hospital and home, helping patients recover with confidence under the care of a specialized, compassionate team.

    How long is a typical stay?
    Most patients stay a few days to a few weeks, depending on their recovery needs and progress.

    Is Transitional Care covered by insurance?
    Yes—Transitional Care is typically covered under Medicare’s Swing Bed benefit. Some commercial insurers may also provide coverage. We’re happy to check your benefits in advance to help you plan.

    How is it different from a nursing home?
    Unlike nursing homes, NBHH provides hospital-based care with immediate access to providers, lab, imaging, and therapy services—all under one roof. Patients benefit from more personalized attention, faster response times, and a higher standard of care.
